11.5 To send a part for repair (RA)

1. If unable to identify part please call, fax or E-mail to Technical Service.
2. If part is known please telephone, fax or E-mail part number and description to
Technical Service for Repair Authorization (RA) number.
3. Clean (disinfect) parts prior to shipping; complete Certificate of Disinfected Parts
Document #40204-17.
4. Ship part with RA number clearly marked on outside of package.
5. Part will be evaluated and customer will be contacted with the cost of repair.
6. After customer approval of repair cost, part will be repaired and return shipped to
customer.
