INSTALLATION INSTRUCTIONS
Plug Valve Lockout

Installation Procedure
1. Before attaching the lockout device, remove the valve handle/wrench
and wipe down the valve stem.
2. Ensure that the valve is in the closed position before locking it out.
3. With the shackle hole of the plug lockout base facing up, position the
plug lockout base over the valve stem, level with the top face of the
valve body.
4. Place the hose clamp over the valve stem, above the plug lockout
base.
5. Applying a minimum of 60 inch pounds (6.8 N m) torque, tighten the screw on the hose clamp with a screw driver until the
plug lockout base can no longer be removed. The base must be sufficiently secured to the valve body to prevent insertion
of a tool under the base of the lockout device to actuate the valve.
6. Insert the hooks of the plug lockout cover into the corresponding slots of the base.
7. Twist the cover counterclockwise to line up the shackle hole of the cover with the shackle hole of the base.
8. Insert the padlock shackle (not included) through the shackle holes and lock the device in place. Do not use locks with
flexible shackles or cable lockouts with this device.
Note: The hose clamp may degrade over time. Verify functionality before each use and replace as needed.
