Warranty and Service Instructions
All requests for return of the Accu-Chek LinkAssist insertion device under the above warranty
must be made to Roche Diabetes Care, Inc. Indianapolis, IN 46256, USA. You will be mailed a
return authorization label, which must be affixed to your carton for shipping the system to Roche.
Cartons received without this label will be returned to you at your expense.
