Use
Anchorages to which personal fall arrest equipment is attached shall be
capable of supporting at least 5000lbs (22.2kN) per person attached, or
shall be designed, installed, and used as part of a complete personal fall
arrest system which maintains a safety factor of at least two, under the
supervision of a qualified person. Failure to do so may result in death or
serious injury.
Positioning pins must always be in place when maneuvering personnel.
Position the cable or rope lifeline directly over the work area. Position
the confined space system on a hard or level surface. Do not use on soft
or loose ground, such as but not limited to, sand, grass, gravel and
rocks. Misuse can result in death or serious injury.
Anchorage hardware must be installed by a qualified person. Anchorage
hardware must be installed according to specification documentation
included with product. Failure to do so may result in death or serious
injury.
