Actions required of you:
1. Please immediately quarantine them and do not distribute.
2. Review and complete the attached Product Retrieval Form. Make one copy of the
completed form for returning product.
3. Fax a completed, signed copy of the form to Jackie Preece at 801-208-3378.
4. Mail the original completed form using the postage paid, self addressed envelope to:
1600 West Merit Parkway, South Jordan, UT 84095-2416
5. Place the copy of the signed completed Product Retrieval Form with the product to be
returned to Merit. The form must accompany all products being returned.
6. Return affected products by shipping them back to Merit via Federal Express 2
(Acct# 1129-5275-6), include the assigned RGA number from the table above on the
outside of the box and ship to:
RGA Department, RGA # (as applicable from Retrieval Form)
