4-A Portable and Infant Bassinet

WARNING
SUFFOCATION HAZARD Infants have suffocated:
• In gaps between extra padding and side of the bassinet, and
• On soft bedding
Use ONLY mattress pad provided by Graco. NEVER add a
pillow,comforter, or another mattress for padding.
• Product can roll over on soft surfaces and suffocate child. Never
place product on beds, sofas, cushions, or other soft surfaces.
FALL HAZARD: Infants have suffered head injuries falling from
bassinet. To help prevent falls,
• DO NOT use this product when the infant begins to push up on
hands and knees, weighs over 20 lb (9 kg) (approximately 5
months of age), or child's head and feet simultaneously touch head
and foot ends of bassinet mesh sides, whichever comes first.
• Child's activity can move product. NEVER place product on counter
tops, tables, or any other elevated surface.
• Bassinet not intended for use as infant restraint devices in motor
vehicles.
• NEVER use this product as a means to transport an infant in a
motor vehicle.
• ALWAYS provide the supervision necessary for the continued safety
of your child.
• NEVER carry child in infant bassinet or toddler playard (with legs
attached).
• The product must be fully assembled per these instructions before
use.
• ALWAYS keep objects out of child's reach.
• NEVER place a child under the product.
• To reduce the risk of SIDS, pediatricians recommend healthy infants
be placed on their backs to sleep, unless otherwise advised by your
physician.
• STRANGULATION HAZARD: Strings can cause strangulation! Do
not place items with a string around a child's neck such as hood
strings or pacifier cords. Do not suspend strings over a bassinet or
cradle or attach strings to toys.
• ALWAYS place child in the center of bassinet.
• Clear floor under bassinet before using as portable bassinet.
• To help prevent heat build-up inside the bassinet and to avoid
overheating your child, DO NOT use an outdoor canopy.
• DO NOT use a sheet with the mattress pad.
