(4) Use the tape measure to help select a prong or a mask. The
selected prong should be large enough to seal the whole of
the nostril. The selected mask should fit exactly next to the
wing of the nose. A prong is pushed onto the Neojet gener-
ator far enough for the curved side of the prong to point to-
wards the patient's top lip. The mask is put on so that it fits
the shape of the nose (Figure 4).
Note: Prongs
Always use the largest-possible prong to avoid leaks and to get
the best possible fit - the lower the leakage, the higher and more
stable the CPAP pressure.
Warning: Prongs and masks
Push the masks or prongs completely onto the Neojet using the
holes provided for this purpose and do not use any cream in the
vicinity of the prongs, masks or Neojets otherwise the prong or
mask may slip off the Neojet (Figures 6 and 7).
The prongs and masks must not be reprocessed and used again.
They are intended to be disposable. Reprocessed prongs and
masks may harden and injure the patient.
(5) Use the tape measure and appropriate color coding scheme
to select a bonnet of the correct size (Figures 3 and 5). The
bonnet should fit well, but not so tightly that it leads to de-
formation of the head. Remove the hook and loop tape from
the bonnet and put it aside where you can reach it easily. The
bonnet is placed on the head so that the hole between the two
fleece parts is central above the forehead. The bonnet should
cover the ears and be pulled down to the nape of the neck.
Thread the securing straps through the eyes on the prong or
mask (Figure 8).
Note about bonnets:
Pull the bonnet down to just above the patient's eyebrows. Bon-
nets which are too small will lead to deformation of the patient's
head. Bonnets which are too big will not secure the CPAP gener-
ator adequately.
(6) Select the most favorable angle for the prong adapter of the
Neojet (45° or 60°). The sharper angle is usually better if you
are using a prong, whereas with a mask, the shallower angle
is better (Figures 6 and 7). Place the Neofoam foam wedge in
the center of the bonnet between the two fleece parts. Fit
the Neojet in the round opening provided for it in the foam.
Guide the prong into the nostrils. Maintain a small distance
between the septum and the base of the prong. When using
the mask, ensure that it is contacting the skin evenly (Figure
8). Leave the nostrils free and do not cover with any part of
the mask. Secure the Neojet to the fleece parts of the bonnet
from the top using the hook and loop end of the strap. Secure
the hook and loop ends of the straps in the area of the ears as
an extension of the fixation loops of the prong. Do not over-
tighten the straps, they are designed purely to provide suffi-
cient hold from the sides (Figure 9).
