4. Lay the anterior vest on patient. The bottom edge should not extend below the costal margin (lowest rib) and
upper edge (center) should be below the sternal notch and centered on the chest. Re-adjust anterior vest height
if necessary always remembering to secure the height with threaded knob. Remove anterior vest and set aside.
VEST APPLICATION
POSTERIOR VEST
Application may require one or two assistants. Ensure upright rods are attached to the posterior vest and slightly loosened.
1. Log roll patient onto their side (Option 1) or raise patient 45 degrees from the waist (Option 2) maintaining
proper c-spine precautions (Figure 6a and 6b).
2. Place posterior vest on patient and return patient to the supine position (Figure 7a and 7b).
Proper Posterior Vest Fit (Figure 8)
A. Upper edge of vest at the level of C7
B. Ends of crossplate in firm contact with trapezius muscles not on the spine of the scapula
C. Superior edges of waist stabilizers at level of xyphoid process
D. Central relief opening in line with spinous processes
ANTERIOR VEST
Ensure upright rods are attached to the anterior vest and slightly loosened.
1. Position the anterior vest on patient's chest so that the lateral superior edge is just below the clavicles (Figure 9).
2. Wrap waist stabilizers and waist straps around patient and insert them through the loops on the anterior vest
(Figure 10). Some trimming of the waist stabilizers may be necessary for smaller patients (Figure 11). (Be careful
not to trim waist stabilizers too short. This can make vest re-application difficult after liner changes.) Sharp edges
as a result of trimming should be rounded. Ensure waist straps are in the alignment tabs to prevent straps from
slipping off of the waist stabilizers. Tighten waist straps and fasten with hook piece (Figure 12).
3. Thread shoulder straps through shoulder loops on the front of the vest. Tighten and secure making sure the
plastic shoulder stabilizers are underneath the straps (Figure 13). Trimming of the shoulder stabilizers may be
necessary depending on the patient size. Alternatively, the plastic shoulder stabilizers may be removed.
Note: All straps should be tightly fastened without causing patient excessive discomfort.
4. Secure waist stabilizers by lining up corresponding holes and applying threaded knob (Figure 14). Threaded
knob must be applied and tightened to properly secure the waist stabilizers. For larger patients where the waist
stabilizers do not overlap in the front, two threaded knobs will be necessary. It may be helpful to note the
number next the hole that the threaded knob is inserted into. This number can be recorded in the patient's
chart for future reference (i. e. during liner changes).
Proper Anterior Vest Fit (Figure 15)
A. Shoulder pieces in close contact with patient's shoulder
B. Shoulder straps secure
C. Vest below sternal notch and centered
D. Superior lateral vest extensions in firm contact with delto-pectoral groove, no contact on clavicles
E. Waist stabilizers cover lower costal margins
F. Threaded knobs are secure
G. Waist straps secure and in alignment tabs on waist stabilizers
SUPERSTRUCTURE
Continue to maintain c-spine alignment throughout superstructure application.
1. Align and attach the ring attachment disk and distraction assembly to halo ring (Figure 16). Make sure the post
on the attachment disk is placed in one of the holes in the "V" portion of the ring.
2. Loosen all upright-transverse clamps and all vest-upright connectors (Figure 17).
3. Insert posterior uprights through posterior upright-transverse clamps and tighten. Insert anterior uprights
through anterior upright-transverse clamps and tighten (Figure 18). Tighten distraction assembly clamp on the
transverse rod (Figure 19).
