Filling in the installation report and warranty passport

IMPORTANT
Please note that any warranty claims can be fulfilled only if the fully and
legibly completed "Installation Report / Warranty Passport" document is
available to Sirona immediately after installation.
1. Enter the serial numbers and software version in the attached
document "Installation Report and Warranty Passport".
2. Complete the remaining points together with the customer.
3. Hand over the copy marked "For the customer" to the customer.
IMPORTANT: In Germany, the copy marked "For the customer" must
be kept in the X-ray system book.
4. Fax the "For the dealer" copy back to Sirona immediately after the
installation.
IMPORTANT: The copy marked "For the dealer" is for the customer
records kept by the dealer.
