Inserting the Tube
8.1

Preparing the Tube

Check the pack content for completeness (D).

For tubes with a cuff, check the cuff (2) for leakage by inflat-
ing it. For this purpose, inflate the cuff with a hand-held manometer
to a pressure of 50 cmH
to see whether the cuff becomes deflated. If the cuff is leak-tight,
remove all air from the cuff with a syringe. Push the cuff upwards
in the direction of the neck flange (5) to facilitate insertion.

Preparing the Patient

Ensure that the patient is well pre-oxygenated before can-
nulation or recannulation.
Slightly overextend the patient's neck, if possible, to facili-
tate insertion.
Should complications arise during cannulation or recannu-
lation, take appropriate safety measures (e.g. tracheal spreader,
tube with a smaller diameter) to allow the doctor to carry out short-
term ventilation using translaryngeal intubation or a larynx mask.
8.3
Inserting the Tube
Carry out the following steps:

Before inserting the tube, remove the inner cannula (3)
and fit the obturator (6) into the outer cannula (1). When inserting
the tube, hold the tube by the flange (5) and press the obturator
firmly into the upper end of the outer cannula with the thumb.
To aid insertion, a thin layer of water-soluble lubricant may be
applied to the protruding part of the obturator at the patient end
and to this area of the tube including the cuff.

Once the tube has been inserted into the patient's tra-
chea, remove the obturator immediately.

Then insert the inner cannula. To secure the inner cannula,
hold the neck flange with the fingertips and twist the connector of
the inner cannula until it locks into place, i.e. until the blue arrows
and/or markings are aligned with each other (C).

Attach the neck strap (12) to the neck flange (5) to keep
the tube in place.

Check the position of the tube as well as its correct func-
tion (see chapter 6 "Warnings"). If required, carefully suction the
trachea and tube to ensure that the airways are unobstructed.

For ventilation, securely connect the standardised 15 mm
connector (3) of the inner cannula to a ventilation machine.
For cuffed tubes:

Inflate the cuff using the pilot balloon (2a). Now check
the cuff pressure to ensure that the cuff has not been damaged

O (≈ 36.78 mmHg) and wait for 1 minute
